New Patients
As part of the process of registering with us we will ask you to attend a “Registration Health Check” with our Health Care Assistant. This helps us to get to know you and your medical history, because sometimes it takes a few weeks for your medical records to catch up with you and arrive here. Additionally, with the benefit of a few basic health-screening procedures such as blood pressure and urine checks, we can give you the best possible advice and care.
To register as a new patient please download the forms below. The completed forms should then be taken in to the practice along with 2 forms of ID – either passport or driving licence and a utility bill. The receptionist will then process your registration and book your appointments.
