As part of the process of registering with us we will ask you to attend a “Registration Health Check” with our healthcare assistant. This helps us to get to know you and your medical history, because sometimes it takes a few weeks for your medical records to arrive here. Additionally, with the benefit of a few basic health-screening procedures such as blood pressure and urine checks, we can give you the best possible advice and care.

We are able to register patients living in Mosborough Village, Westfield and Halfway. Our catchment area is highlighted below:

To register as a new patient please download the forms from the above tab "Download Registration Forms". The completed forms should then be taken in to the practice along with 2 forms of ID – either passport or driving licence and a utility bill. The receptionist will then process your registration and book your appointments.

Medical treatment is available from the date of registration. Please contact reception for further information.
