What is the online form?

Everyone contacting us needs to provide some information on the reasons for need an appointment, and this is triaged by a GP before an appointment is made.

How does it work?

The GP will look at the information provided and make a decision about how to best address the issue.

Why are we doing this?

We want a fairer system for everyone and to improve access to our services. It will give you more flexibility and choice, and ensures the right patient is seen by the right person, at the right time. It also avoids the 8am rush on the phones. 

What is the system called?

We will be using a system called Accurx Total Triage so you may hear some of the team calling it that.

How is information given to the Doctor?

By completing a form via a unique link on our website, the NHS app or a link sent to your phone. If you are unable to fill out the link, please call reception and they can complete it on your behalf.

How will this benefit me?

Every patient query will be looked at by a GP. It will be much quicker and easier to complete the online form than calling us.

What if i can't use an online link?

You can still telephone us or visit us and one of our team members will complete the online form for you. Family members can also complete the form on your behalf.

How can i find more information?

Nurse appointments

Nurse, annual review and blood test appointments can be requested via the online form, or by calling reception. Routine appointments are available with a nurse at various times throughout the day. Demand for some appointment times can be high and you are advised to book in advance. Please inform the receptionist of the reason you wish to see the nurse at the time of booking, to ensure that adequate time is allowed for your appointment.

Hub appointments

What are hub apppointments?

There are satellite hubs across Sheffield that offer GP appointments with ‘extended access’. These are located at many practices across the city, including Darnall Health Centre, Crystal Peaks Medical Centre and The Mathews Practice. Remote and face-to-face appointments can be made with a variety of GP practice staff including Doctors, Advance Care Practitioners, Nurses, Nurse Associates, Health Care Assistants, Physiotherapists and Pharmacists.

Appointments run from 6.30 pm to 9.30 pm during the week, and between 9 am and 6 pm on a Saturday. This may suit your needs better if you are working during the week and can sometimes provide you with access to a GP or healthcare professional quicker. 

You may also be booked into a Hub appointment if Mosborough Health Centre has reached capacity, and you want to be seen that day.

Not in Sheffield?

If you are not in Sheffield, for example for work or holiday, we will ask you to attend a local service instead. We will not manage acute issues over the phone or send acute prescriptions to pharmacies outside of Sheffield. You can access local healthcare by registering as a temporary resident at a local GP, visiting the NHS 111 website, or by calling 111. 

Cancelling an appointment

We ask you to arrive 5 minutes early for your appointments. If you arrive late the clinician may not see you. We are strict about this because lateness can impact other patients and their safety.

If you can’t attend an appointment, we ask you to cancel as soon as possible so it can be offered to someone else.

If you cancel with less than 30 minutes notice, you will be marked as a did not attend (DNA).

You can cancel by:

  • Using the link in your reminder text message (up to 1hour prior to your appointment)
  • Calling the surgery and selecting the options to cancel an appointment and leave a message on our mailbox.
  • Emailing syicb-sheffield.reception.mosboroughhc@nhs.net

 

PLEASE NOTE – any queries left on the email or mailbox that do not relate to cancelling appointments will be deleted and unactioned.

Training Practice

The Health Centre is involved in training medical and nursing staff before and after their professional qualification. You will be informed if a medical student is with the Doctor or Nurse at the time of your consultation and if you do not wish them to be present, please say so. It is perfectly acceptable for you to ask not to have a student present, and you will not be expected to justify your position.

We are also training GP registrars who are qualified doctors training to become GPs. As part of the ongoing training and assessment for the consultation skills of the GP registrars, patient consultations are occasionally recorded using video technology. You will be informed on booking an appointment, and further information with consent forms will be available when you attend the practice. You do not have to consent to being recorded if you do not wish to be.

Did not attend (DNA)

The NHS takes failures to attend appointments very seriously. Last year alone we had over 1,200 wasted appointments.

Repeatedly failing to attend booked appointments are a significant waste of NHS resources and we have a policy in place that runs every 12 months to ensure this is reduced. Each did not attend is documented in your patient record. Please see our policy below:

1) After one DNA we will send you a text message to remind you of our practice policy.

2) After two DNAs, we will send you a warning letter.

3) After a third DNA we may arrange for you to meet with the practice manager, and we may remove you from the practice list.

If you feel you have been sent the above incorrectly, we are always happy to discuss it.

Local Pharmacy Services

Pharmacy First Scheme

If you have symptoms relating to any of the seven common conditions below, you can obtain a free NHS consultation at
your local pharmacy without the need for a GP appointment. 

  • Sinusitis (for patients aged 12 years and over only)
  • Sore throat (aged 5+)
  • Earache (aged 1 year-17)
  • Infected insect bite (aged 1 year+)
  • Impetigo (aged 18 years+)
  • Shingles (aged 18 years+)
  • Uncomplicated urinary tract infections (women aged 16 to 64)

 

Blood Pressure Check

Why get your blood pressure checked?

  • High blood pressure, also called hypertension, is a condition which can be controlled to reduce your risk of a heart attack, stroke or other cardiovascular disease.

  • In the UK there are about five million adults (one in every nine) who have high blood pressure without even knowing it, since high blood pressure itself rarely causes symptoms.

  • The British Heart Foundation estimates that high blood pressure causes over 50% of heart attacks and strokes.

What does this free NHS blood pressure check involve?

  • Pharmacies are offering free NHS blood pressure checks to people aged 40 and over with undiagnosed high blood pressure. No appointment necessary.

  • This involves around 10-15 minutes in the pharmacy consultation room with pharmacy staff. Following this, you may be invited to take home a blood pressure monitor that measures your blood pressure as you go about your daily life.

  • Depending on your blood pressure reading you may be referred to your GP. The pharmacy staff will guide you through any necessary steps depending on your blood pressure result.

Contraception Services

Need a free supply of oral contraception or Emergency Contraception?

If you need Emergency Contraception, are already taking oral contraception (the pill) or are looking for your first supply, you can arrange a confidential consultation at a local participating pharmacy.

Simply walk in to the pharmacy and ask about their contraception service. Your GP or sexual health clinic can also refer you, but you don’t have to be registered with a GP.

Your one-to-one consultation with a pharmacist/technician will be held in a private room in the pharmacy. If they agree that the pill is suitable for you, a free supply can be made by the pharmacy.

The pharmacy contraception service is confidential and free for everyone. Unless you give your permission, the pharmacy will not tell anyone about your consultation with them, including your GP.
